Once you click on the form, you can type in your information but you must SAVE to your computer and then email back to the office at mmorelli@saintadalbertca.org You only have to complete one of each form for a family.

1. Complete the Form (It’s fillable, so you can type in it)

2. Save to your Computer (MyDocuments, Downloads, etc.)

3. Email the attachment to mmorelli@saintadalbertca.org
