International Food Fair Survey

We would like to explore the possibility of having an International Food Fair during Catholic Schools Week (Jan 30 - Feb 2). In order to make this event a success, we would need as many families to contribute a dish from their culture. Can you please fill out the following form which would help us with the planning.
